What do I set the CSV Header rows to?
CSV headers for premium tablerates can be set to anything you like. The CSV headers are not used for processing and just there to describe the columns to you. However, make sure the data in the columns matches the data with the CSV definition.

Both of the above mentioned CSVs are valid and define the same rule, with different headers.
